1. Transcript, report card, or withdrawal papers

2. Original birth certificate or passport

3. Photo ID of parent or legal guardian
    a. Notarized guardianship papers (if applicable)

4. One proof of residence: (notarized residential affidavit if applicable)
    a. Electric, water or gas bill
    b. Residential lease
    c. Mortgage contract or payment coupon
    d. Rental agreement

5. Immunization record:
    DPT/DT 4 required MMR 2 required
    Hepatitis B 3 required Polio 4 required
    Hepatitis A 2 required Chickenpox 2 required

Unacceptable Bills for Registration:
Cable, Drivers License, Telephone and Disposal
